Q. Why do Japanese enter onsen tubs with folded towels on their heads?

A. As some onsen water is drinkable, it is good etiquette for bathers to keep the water clean. Japanese bathe naked when they take an onsen or a bath. In Japan, nakedness is considered the cleanest state, so they wash off their sweat and grime with hot water before entering the tub. It is bad manners to wear a swimming suit or underwear while bathing. .

When you take a bath you may take a towel with you. It is a Japanese custom to enter the tub putting your folded towel on your head so that you do not leave it anywhere in the bathroom. However, there are increasing numbers of people who do not know these customs and elderly people are often disgusted to see them. On the other hand, heartwarming scenes of foreigners with a folded towel on their head are sometimes seen.

Needless to say you must wash your body outside the tub and never do washing in the bathing room.
